I think she was trying to make a joke about how Germans control North Dakota because everything on the map is changed but the city of Bismarck. So the idea is Germans already control it. It doesn't really work because everyone already knows these cities are named by the people who settle them so it isn't to far off to say Germans do in a way control North Dakota.

Nah, that's a Pippa lifestyle choice and has nothing to do with her dentist visit. If you grow up poor you live like you're poor when you're an adult, even if you don't have the same money problems. It's a hard mindset to break out of, even when it's a disadvantageous one.
