I'd guess it's a mix of rotoscope and scenes with no meat parts with the model edited over it after. That's why some items pop into existence.

For static images it's probably an image over an image to make layers easy.

I think the hand using the scraper is angled down based on how the scraper is moving and the animated hand over it is so big that it's easy to cover the meat hand with it so much that there's no chance of exposer. The meat and virtual hands are in different positions, meat being angled in a way to be narrow and animated to be wider.
