Some mainstream ones:
Doukyuusei and it's sequels - Really sweet manga that fumbles a bit in the middle parts but has one of the best endings in my opinions. Has an anime movie that's really good.
Umibe no Etranger - Really good manga that is a bit slow at parts. New chapters are sadly taking forever to come out. Has a good anime movie.
Given - The one that most people know. Suffers from "Everyone is gay-itis" in the later volumes where all the side characters also just happen to be gay. Really good anime with one of the strongest single episodes.
Sasaki to Miyano - Very good, both the manga and anime. Good spin-off with Hirano to Kagiura as well.
Short but sweet:
Konbini-kun - Everything from Junko is good, sadly she found success with regular shojo so she doesn't do BL anymore.
Nagahama to Be, or not to Be - Scarlet Beriko also only has bangers, most on the shorter side.
Longer series:
Blue Sky Complex - One of the best I've ever read and it keeps up the quality throughout.
Trashy smut:
Too many to list, best pick for an author to start with would probably be Owal or some manga by Ogeretsu Tanaka (some of hers are turbo depression though)
And Dick Fight Island as already discussed.
This is just Japanese manga and there would be a whole 'nother list for Korean and Chinese stuff.
