Assuming she caught a fever/cold relatively recent then this is pretty normal. Fever/cold do come along with internal bleeding and vice versa, that's why there is always antibiotic in the pack of stuff. Ask the nearest person you know that caught fever or cold seasonally and they'll most likely tell you they got worse blood from coughing.

I've been cursed to deal with sick people for half my life at this point and no one who has ever gotten a cold, even a bad one has ever coughed up blood. If anything that comes out of your body is mixed with blood you're spending a day in the emergency room at the very least. Hopefully it's just a bad case of bronchitis due to a badly treated infection or lack of treatment in the case of the average phase girl, either way they need to run tests to make sure.

Some cold do follow up or associated with a slight fever, especially more common if the patient's body is fucked up in some way (something natural resistance something).
Anyway, it's most likely isn't something serious that left the average phase girl in a do or die situation. You can naturally get better from being sick, but that will accumulate some blood somewhere from the likely internal wound (either bleeding or inflammation), which, when shit hits the fan, enough force will blew up all the hidden stored blood.
