Webbed toes can be for a lot of reasons, some more benign than others.
Also look how he stands in his shoes, pressing outwards, that is very unusual and the kind of thing people pick up on passively when interacting with others that indicates to them something is off.
Also when looking at a more candid photo of this guy he doesn't look particularly normal. He doesn't look disgusting right off the bat, but as you stare it gets worse. Mouth naturally curves into a frown, upward lift of the nose, eyes naturally curve downward, blotchy skin, double chin, gaps in his facial hair.

Naw Mr. ESL, they can be from like incest which has a great deal of comorbidities or from being born a few weeks premature (toes are one of the things that grow last).
